Transcript Request for Current Seniors - Four Year College Application Tracking Form for Olympia High School

Remember to use an envelope and fill out the college address. No postage is necessary. Do NOT put YOUR return address on the envelope. We will stamp in the OHS return address.

Transcript Request for Freshman, Sophomores, Juniors, and any student not currently enrolled at OHS(Past Grads, Non-Grads):

Please fill out the form completely, including $2.00 cash or check made out Olympia High School. Allow forty-eight hours to process.
