Athletes may clear ONLY on their sport specific date and "Open Clearance Dates"
Any Athlete not cleared by 5:00pm on November 12, WILL miss the first day of practice.
Starting November 16, athletes will not participate until the day after all forms are turned in.
What you need if you already cleared for a Fall sport this school year:
• Have current physical on file. Initial Physical Physical Update
• Sports Specific Safety Guidelines -- see below
• Concussion and Head Injury Acknowledgement
• Pay $125 participation fee.
If turning out for a cut-sport (boys and girls basketball), you won't pay participation fee until you make the team.
What you need for your first sports clearance of this school year:

D. If turning out for a non-cut sport, pay $125 participation fee.

E. Pay all fines at ASB Office.
F. If you are a private school, home school, or Running Start student, call the athletic office you may have additonal paperwork.

How the Clearance Process Works:
Students will first go to the ASB Office and purchase an ASB card and pay the “Play to Pay” fee. They will check to see that all fines are cleared. Students will then go next door to the Athletic Office where the packet will be reviewed and collected. They will make sure all signatures are complete, insurance information is available and physical exam documentation is up to date. They will also check student’s grades from the previous grading period. Students must have passed all their classes or they will be placed on academic probation for the first five weeks.
Grades
Olympia School District Policy requires a
passing grade in all subjects in order to be fully eligible for
interscholastic activity.

you are new to OHS or a freshman, home schooled, or private schooled ,
please bring a copy of your report card/transcript from last semester.
Pay to Play Fee
The "Pay to Play" fee is $125 per athlete, per sport. There are no discountes for families or students who participate in more than one sport per year.The "Pay to Play" can be waived only if an athele is on the free and reduced-price meal program.

Refund Policy for Athletes Who are Cut or Quit -
Athletes who are cut from a team will automatically begin the refund process. Refund checks are distributed from the Olympia School District Offices and can take up to a months time, depending on when the next School Board meeting is scheduled so monies can be approved.
If a student leaves the sport within the first ten (10) days of practice 100% of the fee will be refunded. Requests for refunds must be made, in writing, to the building athletic office within five (5) days of leaving the sport. If a student leves the sport after the first ten (10) days of practice no refund will be given.

Athletic Forms In Detail
Athletic Registration Form --- Insurance & Assurance:
Each year students and parents are required to provide information
related to residence, insurance coverage and permission to
participate. The "Athletic Registration" forms must be complete and
returned during the initial clearance period each school year. (You can fill out this form and then print it, signatures required on back)
Student/Athlete Contract:
This contract states that students participating in
athletic/activities programs has read, understands and will abide by
the Olympia School District Athletic Code for High School Students
found in policy 3200. Students along with parents/legal guardians must
read, sign and return this contract during their initial clearance
period of each school year.
Physical Exam:
Prior to the first participation in high school athletics, students must undergo an initial physical examination and be approved for interscholastic athletic competition. The "initial" physical examination is valid for a period of 24 consecutive months with an update conducted after 13 months. A medical authority, licensed to perform a physical examination, must sign the form which must be returned during the clearance period. Only forms containing original signatures will be accepted; faxed and/or copied forms will not be accepted.
Incomming freshmen must have a new complete physical dated after April 30, 2009
Returning athletes may call the Athletic Office at 596-7028 and ask what the physical date is on file.

Medical Travel Card:
Injuries and illness may occur at practices or games when
parents/guardians are not immediately available. The "Medical Travel
Card" provides school staff with information directly related to health
conditions, parent/guardian notification process, along with choices of
physicians and hospitals. This form must be completed, signed and
returned during the clearance period for each season.

Concussion and Head Injury Acknowledgement
The "Concussion and Head Injury Acknowledgement" must be completed, signed and returned during the initial clearance period each school year.
Assumption of Risk and Release of Liability
The "Assumption of Risk and Release of Liability" must be completed, signed and returned during the initial clearance period each school year.
