Sport Clearance for Fall 2008
Due to changes in the athletic policy and the uncertainty of the pay to play fee, complete sports packets will not be available until after the first week in August.
You can pickup physical forms by the front reception desk or download some of but not all of them from the links below.
Clearance dates:
FOOTBALL -- Clearance August 18 & 19 from 9:00-11:00am & 12:30-3:30pm --
If a football player does not clear on these dates they will miss the first day of practice on August 20,2008
ALL OTHER FALL SPORTS -- Volleyball, Cross Country, Girls Swimming, Boys Tennis, Girls Soccer
Clearance is August 21, 22 from 9:00-11:00am & 12:30-3:30pm --
If an athlete does not clear during these dates... they will miss the first day of practice on August 25, 2008
Sports Physical Requirements for High School
All students must submit an Initial Athletic Physical Examination Form prior to participating in sports for the first time at the high school level. The full physical exam is good for 13 months. At that time a Physical Exam Update Form must be completed by the physician to extend eligibility for the second year. A new complete physical exam must be done two years after the initial exam.
You may call the Athletic Office at 596-7028 and ask what the physical date is on file.
Original signatures and forms are required, Please -no faxes or Xerox copies.
Grades
Olympia School District Policy requires a passing grade in all subjects in order to be fully eligible for interscholastic activity.
Students must have passed all their classes or they will be placed on academic probation for the first five weeks.
Sports Clearance
All athletes wishing to participate in a sport must submit a completed sports clearance packet.
You will need the following
- a. Student-Athlete Checklist Form
- If you answer yes to any of the questions there may be additional paperwork at time of clearance.
- b. Green Athletic Registration Folder (Pick up in commons or front reception area)
- c. Physical Examination Form (Initial, Update)
- (good for 13 months)
- Original signatures are required, no faxes or copies.
- d. Blue Emergency Medical Card (Pick up in commons or front reception area)
- e. Sport Specific Liability Form
- f. Attendance Policy Form
- g. Current school year ASB card - $40
- h. Pay to Play fee - (currently at $90 per sport -- this fee could increase before August clearance depending on school board decision)
- i. All fines must be cleared or paid at the ASB office
How the Clearance Process Works
Make sure to bring all the completed forms and items listed above.
There are several places for parent and athlete signatures, please check forms carefully.
1.) Athletes will first line up at the ASB office and enter one at a time. At the ASB office athletes will purchase an ASB card ($40) and pay the Pay to Play fee ($125) Any outstanding fees or fines must be cleared at this time.
2.) After athletes have been to ASB office and received the needed signatures, they will then proceed to the Athletic office (located next door to the ASB office).
She will review the forms to make sure all signatures are complete, insurance information is available and physical exam documentation is up to date. She will check student’s grades from the previous grading period.

If the student did not attend OHS last year and are not coming form Reeves or Washington Middle School, you will need to bring a copy of your last report card.
